Hi Karen! As mentioned I am looking for complete honesty and I know you'll deliver. I sent you a questionnaire via email. Its purpose is to get a conversation going and I will follow up with more questions.

I would ask that you not share or discuss the story with family, friends and colleagues. There are copyright issues. It has not gone through final copyediting.
